I was wondering if anyone could tell me what would cause a water pump to fly apart? It came off and sent the fan into the radiator. the threads on the water pump where the fan goes on broke off and actually some of them stuck inside the threads of the fan. if that makes sense.

So, the water pump itself did not actually fly apart? The w.p. didn't actually come off either did it?

Hard to tell from your post but it sounds like the fan came off the w.p. Is that correct?

Uh, somebody replace this pump, or work around fan recently?

What it sounds like is that bolts/studs were over-torqued when installing fan and subsequently let go.

The bearings can go in the pump, but with bearing failure you usually get plenty of warning before you get catastrophic failure such as you had.

Don't know, but it sounds like installation error to me.
