My rear window fluid worked until a month after I bought the van. Is there a separate reservoir for the rear glass wash fluid and if so, where is it located?

Yah, they're in the same resevoir, but if you look at it, the pump for the rear is located higher in the tank than the one for the front washers. I think this is to make sure theres always fluid for the front. When I got my montana, the rear didnt work but the pump made noise. After looking at it, the hose was not connected. I just pushed it back together and voila, working rear washer fluid! The pumps are like 15 bucks at autozone, so if it don't make noise or if the resevoir is full, hose connected, and it makes noise, you need to replace the pump. They just pull out of the resevoir tank and unplug from electrical connection and hose. i had one die on my Lumina (car) and you couldn't tell it was broke till I took it out of the tank and the bottom (where it sucks up the fluid) was broken off.

LOL
Yeah, this freaked me out when I first got our van too. Like the others said, one resevoir serviced both windows, with the rear sitting higher and consequently stopping working first.
