changing out inner tierod

So I need to change out my inner tie rod because it has play and causes the passenger side tire to wobble at speed. I bought the tie rod last month and was all set with my haynes manual to do the job, when I noticed that the haynes manual only says how to replace the outer end. no direction whatsoever for the inner. I know that if the outer is disconnected the inner just screws on, but heres my question: There does not appear to be any flat spots on the rack piston end to brace it with a wrench while unscrewing the outer tierod end from it. There is a flat spot(s) on the rod end to put a big wrench on (though I don't know how in the world to snake a wrench up in there). I'm afraid i'll break something in the rack and pinion if I just unscrew the tierod end (which is probably pretty tight).
Am I missing something here? I noticed a small (very small) threaded hole in the end of the pinion where it meets the rod end, but its very small, like I could maybe put a small allen wrench in there to hold it while I unscrew, but its so small, if the rod end is tight it would just bend the tiny allen wrench. So, after all this long cajolery, does anyone have any suggestions or has anyone here done this repair themselves?? Help would be much appreciated!!

What year is your van? Before you do anything, make sure it's not on the recall list. Call a dealer and give tehm your vin, and they can tell you if your van is covered under the recall notice. If your problem isn't recall related, it takes a special tool to remove inner tie rods. You may be able to rent one free at Auto-zone, if they have that tool for rent.

I'm checking with the dealer today about recalls. I didn't know there was one for the tierod? I'm hopeing they can fix my shuddering power steering though. Anyways, i know its the tie rod because i've taken the tire off and removed the boot that covers the inner rod and can see thats where the play is. Autozone has a tool (made of ABS plastic) that aids in screwing the tierod back together but nothing to take the innner one out, as it is supposed to just unscrew once the outer tie rod has been disconnected or loosened (with both tires off the ground so as you unscrew it just move the passenger side out of alignment).
Has anyone done the inner tie rod themselves?

the inner tie rod is included on the rack, therefore would be covered under the recall.

Well I did the Tie Rod yesterday. Autozone has this nifty tool called a Saginaw tierod remover. Its a specially machined unit that slips over the tierod and grips on the flat spots, and then connects to another long shaft with a socket plug on the end. It probably took 200 ft/lb (my dad standing on the ratchet) to break it free, but then it came off easily. Used the tool to reinstall it, and then got an alignment (after getting the front wheels as straight as possible myself before tightening the jam nut)
