"Cat back" refers to replacing the exhaust system after the catalytic converters. Usually, this is a pipe, the mufflers, and the exhaust tip. Other options are an axle back which is just the mufflers and tips and a full exhaust system, which is from the exhaust manifolds (or headers) all the way to the tips. This is legal only on cars without catalytic converters (1976 and earlier) in most states.

It's possible to get a full exhaust system with Hi-flow Cats as well... that would be the ideal setup for a emissions controlled car.
