'95 Plymouth Neon Error Codes

Im getting These codes in my 95 plymouth neon highline.

12 - Direct Battery Input was disconnected in last 50 keyons
25 - Idle air control motor circuits problem, target idle not reached(+/- 200), vacuum leak found
41 - Generator field not switching properly


Im not worried about 12, as my wife left the lights on the other day and killed the battery.

I noticed a loose hose under the hood, connects into the large host that goes from the air filter to the manifold, could that be causing the 25 error? How would i test to make sure i dont still have an air leak.

I am at a loss as to what error 41 means. Any ideas?

P.S. Anyone know how to get the OBDII Checker to work on this car? I keep trying with a AutoXRay EZ-Read 2000 (http://www.autoxray.com/products.aspx?sub=readers&id=33) which says it supports my car. its database correctly identifies the position of the sensor, but does when i put in my car info, it is looking for the old style connector, not the OBDII connector, and it displays "not responding". If i put it on Generic OBDII, it will say its initializing, flash 2 or 3 times, and then it just dies. I have to take the batteries out to get it to come on again.
